We provide behavioral health, suboxone, dental, OB/GYN, women's care, medical, pediatrics, and podiatry. We have nine locations and all can be found in Mansfield or Shelby, Ohio. Our main location is located at 600 West Third Street, Mansfield, OH 44906.

Patient Portal incorporates the most basic of services, such as updating patient demographics and appointment requests, to the more complex services, such as submitting “Ask a Nurse” questions.

  • Update demographic information

    Existing patients are able to view demographic and guarantor information residing within the system and, if needed, submit changes via the web. Patients are able to select a primary pharmacy and designate primary and emergency contact(s).

  • Submit appointment requests

    For non-urgent appointments, Patient Portal allows patients to request an appointment. The patient is able to select their preferred provider, date range and time spans and the reason(s) for the appointment. Since this is a request for an appointment the patient will be contacted by phone to schedule an appointment within or near the date range and time spans they requested.

  • Ask the nurse a question

    Allowing patients to submit non-urgent questions via the web can reduce the number of incoming telephone calls.
